NN5 + nikon D300s + 70-200 2.8 vr I

HI all

i tried to set the 70-200 + D300s on my NN5
but i can't find the NNP

i know that i have to change both rails
and to put the camera plate on the lens

and that's it

did any one did it befor and can write me the numbers?
