    The little locking device that holds the tripod screw in place is a little square metal plate with protruding edges on two sides that slide along the tripod screw track and engage the T-II when the locking screw is tightened.

    I have noticed that sometimes that little square plate can turn 90 degrees putting the protruding edges in a position where they cannot engage the T-II. In this position, it will sit in the grove at an angle and prevent the camera from seating properly onto the T-II.

    I suggest either putting the protruding edges on all four sides so that if it does accidentally rotate 90 degrees it will still engage properly or making the little square plate rectangular so it can't twist at all.
