How do you do a pano out of the side of a skyscraper?

I'm looking into the idea of making a pano at the top of a 60 mertre scaffolding tower. I saw a pano once with a floating effect from the window of a skyscraper in New York, it appeared as if the pano was taken about 4m out. How is this effect done?

It was probably done with the camera on the end of a pole held out of a window.

Ok, I thought so, does that mean that done with a 8mm fisheye and just twisted around a few times regardless of nodal point etc. Stitching done manually?
